I hereby acknowledge and agree to the following terms and conditions in consideration of being allowed to participate in dance, voice, piano, yoga, or any other classes, workshops, rehearsals, performances, as well as activities and events held at Studio de Myriam: *Assumption of Risk: I understand and acknowledge that participation in dance, voice, piano, yoga, or any other classes, workshops, rehearsals, performances, as well as attendance at activities and events, involves inherent risks and hazards. These risks may include, but are not limited to, physical exertion, bodily injury, falls, collisions with other participants, equipment malfunction, and any other foreseeable or unforeseeable risks associated with the activity. Release of Liability: I hereby release, waive, discharge, and covenant not to sue Studio de Myriam, its owners, instructors, employees, agents, students, or volunteers (hereinafter referred to as “Released Parties”) from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, my child, or any property belonging to me or my child while participating in dance, voice, piano, yoga, or any other classes, workshops, rehearsals, performances, or while attending activities and events at Studio de Myriam. Indemnification: I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost, including attorney fees, that may incur due to my participation in dance, voice, piano, yoga, or any other classes, workshops, rehearsals, performances, or while attending activities and events, whether caused by negligence of the Released Parties or otherwise. Medical Treatment: I authorize the Released Parties to seek and consent to emergency medical treatment or services for me in case of injury, accident, or illness during my participation in the classes or attendance at events. I understand that I am solely responsible for all costs incurred due to any such medical treatment or services. Agreement Not to Sue: I agree not to initiate any claim, lawsuit, court action, or other legal proceedings against the Released Parties, nor join or assist in the prosecution of any claim for money or other damages that I might have due to my participation in the classes or attendance at events. Acknowledgment of Understanding: I have read this waiver and fully understand its terms. I am aware that by signing this document, I am waiving certain legal rights, including the right to sue. If I am signing this waiver for my child, I certify that I am the parent or legal guardian and have the right to waive these rights.
